Please complete the form as legibly as possible and bring it to your initial Estate Planning consultation.

You are welcome to bring account statements, deeds, titles, corporate/LLC books and forms, 

and other important documents to our meeting if you'd like.

On average, an initial meeting for Estate Planning will take an hour.
